【来源】《仁斋直指》卷二十。
【组成】沙苑蒺藜(形如羊肾，慢火略炒，杵去刺)防风羌活甘草(盐水炙)石决明(捣碎，研，水飞过)草决明蝉蜕(去足)蛇皮(剪碎，和麻油，新瓦炒)川芎各15克甘菊
【用法】上药为末。每次4.5克，用麦门冬(去心)煎汤下，食后临卧服。
【功用】去风消翳。
【主治】诸风上侵，眼生翳膜。
